Abstract

Breast cancer (BC) is a heterogeneous disease classified into five intrinsic subtypes: luminal A, luminal B, HER2-positive, triple-negative breast cancer (TNBC), and normal-like. These subtypes differ not only in receptor expression but also in immune profiles, including tumor-infiltrating lymphocytes (TILs), PD-L1 expression, tumor-associated antigens, and tumor mutational burden (TMB). Notably, no BC subtype has been identified as immunologically quiet, suggesting the potential for immunotherapy to harness the host immune response. Although BC immunotherapy research initially lagged behind that of other solid tumors, clinical trials investigating immune-based treatments for BC have expanded rapidly. Unlike conventional drug development, these trials are being conducted simultaneously in both metastatic and early-stage disease settings. This shift is driven by the hypothesis that BC tumors become less immune-sensitive over time, as reflected by declining TILs and reduced immune signatures in metastatic samples. This review provides a comprehensive overview of ongoing clinical trials exploring immunotherapy for early BC, discussing trial designs, combinatorial strategies, immune-related biomarkers, and the potential advantages and challenges of early-stage immunotherapy implementation. Breast cancer is one of the most common malignancies affecting women worldwide, and chemotherapy plays a crucial role in its treatment. Chemotherapy is used in various settings, including neoadjuvant (before surgery) to shrink tumors, adjuvant (after surgery) to eliminate residual cancer cells, and metastatic cases to control disease progression. Common chemotherapeutic agents include anthracyclines (e.g., doxorubicin), taxanes (e.g., paclitaxel), and platinum-based drugs, often combined for optimal efficacy.
